脱敏脱敏desensitization治疗Ⅰ型变态反应病常用的一种免疫疗法。又称减敏。用小量渐增的方法给机体引入导致患者发生变态反应的变应原，使机体产生免疫耐受性。Ⅰ型变态反应是由免疫球蛋白E（IgE）和肥大细胞介导的速发型变态反应。变应原与肥大细胞上结合的IgE作用，使肥大细胞释放介质，引起临床反应。实验证明，进行脱敏治疗后，血清中IgE和免疫球蛋白G（IgG）的水平逐渐上升，到约4个月时，IgE水平开始下降，而IgG的水平则继续上升，到治疗结束时，其水平可达治疗前的千倍。IgG不能与肥大细胞结合，它在血液中可随时捕捉进入的变应原，从而保护了肥大细胞，阻断了临床症状的发生，所以称为阻断抗体或封闭抗体。脱敏治疗的机理比较复杂，有些至今仍不清楚。脱敏治疗一般采用皮下注射。初次接受治疗时要从低浓度、小剂量开始，逐步递增浓度和剂量，以达到患者所能耐受的最高浓度和最大剂量；然后把浓度和剂量固定在此水平，再重复注射一定时期。注射一般每2～3天进行一次，总的治疗时间约需2～3年。治疗结束后，每隔数年还需进行短期的加强注射，以巩固疗效。脱敏治疗对Ⅰ型变态反应，特别是呼吸系变态反应（花粉症、常年性变应性鼻炎、支气管哮喘等）效果较好，可以达到预防发作的目的；而其他对症疗法只能暂时控制症状，不能预防复发。最大缺点是疗程太长，患者难以坚持。为此，近来有采用化学修饰变应原和快速脱敏的，其目的都在于减少注射次数，缩短疗程。脱敏治疗一般是安全的。如果因为剂量过大导致不耐受，多表现为原发疾病的症状加重，此时适当调整剂量或暂停注射，多能缓解，以后仍可继续注射。因全身或局部严重反应需中止治疗的情况极少发生。
